Varicocele Evaluation

Varicocele is a common condition of the male reproductive system and is characterized by dilation of the veins of the spermatic cord. It occurs in a significant proportion of men of reproductive age and, in some cases, may affect spermatogenesis.

Evaluation for varicocele is part of the investigation of male infertility when there are abnormal findings or a clinical suspicion that this venous disorder may be affecting fertility.

What Varicocele Is and What Causes It

Varicocele is the abnormal dilation and tortuous enlargement of the veins that drain blood from the testicle. It is most commonly found on the left side due to anatomical особенities of venous drainage.

The main cause is venous reflux, meaning inadequate function of the venous valves, which results in blood pooling around the testicle. This condition may lead to disruption of the testicle’s temperature regulation, impaired microcirculation, and increased oxidative stress.

The presence of varicocele does not necessarily mean an inability to father children. Many men with varicocele maintain normal fertility and have children without intervention.

The effect of varicocele on fertility depends on:

  • The severity of the venous dilation
  • The findings of the semen analysis
  • The duration of infertility
  • The partner’s age and fertility status

In cases of mild varicocele with a normal semen analysis, treatment is usually not required. In contrast, when there is an abnormal semen analysis or persistent infertility, varicocele may be a contributing factor that needs evaluation.

Treatment for varicocele is not indicated in every case. The decision is based on its association with abnormalities in the semen analysis or with symptoms.

Surgical treatment is considered in cases of infertility with an abnormal semen analysis or when varicocele is accompanied by persistent pain and a documented impact on fertility.

In mild varicocele without laboratory abnormalities, monitoring is usually recommended.

Varicocele and IVF

The presence of varicocele alone is not in itself an indication for in vitro fertilization (IVF). The decision depends on the severity of the condition, the duration of infertility, and factors related to the woman.

In some cases, surgical repair of varicocele may precede assisted reproduction, particularly when there is a severe impairment of spermatogenesis and there is time to wait. In other cases, when the woman’s age or the overall circumstances do not allow delay, IVF may be recommended directly.

How Varicocele Evaluation Is Performed

Evaluation for varicocele begins with a clinical examination by the physician.

To confirm the diagnosis and assess severity, a scrotal ultrasound is performed. This examination allows:

  • Visualization of the venous dilation
  • Recording of venous reflux
  • Assessment of the size and morphology of the testes

Varicocele and the Reproductive Journey
